DS-156 Non-Immigrant Visa Application form (EVAF)

The State Department has released an electronic version of the DS-156 Non-Immigrant Visa Application form.
This form must be filled out by all applicants accessing a specific webpage, which will open up on a new browser window when you click on the link above. After inputting all required information, applicants must download and print a completed form, which includes a bar code on the third page. Please follow the instructions carefully or you will not be able to download this form. At this time, this form is only available in the following languages: English, Spanish, Korean, Polish, French, German, Japanese and Portuguese. We cannot accept a faxed copy of the form. After printing the three pages you must follow the instructions on how to apply for an appointment. You must also make sure you have the correct documents for the visa type you are requesting.

DS-157 Supplemental Non-immigrant Visa Application Form

DS-157 Supplemental Non-immigrant Visa Application Form should be filled by all male non-immigrant visa applicants, between the ages of 16 and 45.
This requirement does not apply to official or diplomatic Visa categories (except for A-3, G-5, and NATO-7).

DS-158 Contact Information and Work History Form

DS-158 Contact Information and Work History Form should be filled by all applicants seeking F, J, and M visas.

DS-156E Non-Immigrant Traty Trader/Investor Visa Application Form

DS-156E Non-Immigrant Traty Trader/Investor Visa Application Form should be filled by all applicants seeking E1/E2 visas.
